Everything you need to know before your first visit.
What to Expect
Your initial visit is 90 minutes and includes a full intake conversation about your health history and goals, followed by your first treatment. Follow-up visits are 60 minutes. Please plan to arrive 5 minutes early so your full session can start on time, and wear loose, comfortable clothing.
Intake Paperwork
New patient intake and consent forms can be completed online before your visit to save time in the office. Set up your account in our Patient Portal to book your visit and fill out your forms ahead of time. If you'd prefer a paper copy or need help completing the forms, just let us know.
Policies
- Cancellations & No-Shows
- We ask for at least 24 hours' notice to cancel or reschedule. Your first late cancellation or no-show is a one-time courtesy waiver at no charge; after that, a $50 fee applies to each late cancellation or no-show, for both single visits and package visits. For package patients, the $50 fee is charged instead of deducting a package visit, so your prepaid visits are preserved.
- Late Arrival
- Sessions end at the scheduled time — arriving late will shorten your session.
- Illness
- If you have a fever, are vomiting, or have contagious symptoms, please don't come in — contact us as soon as possible. Where appropriate, we'll switch your visit to telehealth; if telehealth isn't appropriate, the cancellation is fee-exempt.
- Payment
- Payment is due in full at the time of service, by cash, major debit/credit card, or HSA/FSA card. A valid card must be kept on file before treatment begins, even if you typically pay another way — it may be charged for amounts not paid at the time of service, including late cancellation, no-show, and returned payment fees. A returned or reversed payment (including a chargeback) carries a $30 fee in addition to the unpaid amount.
- Insurance
- Lively Vibe is a cash-pay practice and does not bill insurance directly. A superbill — an itemized receipt with diagnosis and procedure codes — is available on request for services you actually attended, which you can submit to your insurer for possible reimbursement. We can't guarantee reimbursement, since that depends on your specific plan.
- Good Faith Estimate
- Because you pay for your own care, federal law gives you the right to a written Good Faith Estimate of expected charges when you schedule, or on request. Our current prices are on the Services page. If your bill is at least $400 more than your estimate, you may dispute it — see cms.gov/nosurprises for details.
- HSA & FSA / Letter of Medical Necessity
- Acupuncture may qualify as a medical expense for HSA/FSA reimbursement under IRS rules. Eligibility depends on your plan, so please confirm with your administrator. On request, we can provide a Letter of Medical Necessity (LMN) reflecting your diagnosis and treatment plan — often helpful for chronic pain, migraines, or stress-related conditions. Approval isn't guaranteed, and this isn't tax advice.
- Herbal Medicine & Retail Products
- Herbs, supplements, and other wellness products are dispensed for your individual care and can't be restocked once they leave the clinic, so all sales are final. If a product is defective, damaged, or recalled, contact us for a replacement or refund. If you have a reaction, stop using it and contact us right away.
- AI Note-Taking
- We may use an AI transcription feature in our health record system to help draft clinical notes. We'll ask for your verbal consent at the start of any visit where it's used, and you may decline or ask that it be turned off at any time, with no effect on your care.
- Minors
- Washington law requires a parent or legal guardian's consent before a minor under 18 can receive care. A parent or guardian must complete and sign all consent forms on the minor's behalf and be present at every visit.
